A missed period, new chin hair, stubborn acne, or difficulty conceiving can raise a reasonable question: how is PCOS diagnosed? The answer is more layered than a single blood test or an ultrasound image. Polycystic ovary syndrome is a pattern of hormone and ovulation-related signs, and a thoughtful diagnosis looks at the whole pattern while ruling out other conditions that can look similar.

That can feel frustrating when you want a clear answer. But the process matters. A careful evaluation helps you understand what is actually driving your symptoms and gives you a better foundation for decisions about cycles, fertility, metabolic health, and day-to-day support.

How Is PCOS Diagnosed? The Core Criteria

For most adults, clinicians use the Rotterdam criteria. After excluding other possible causes, a PCOS diagnosis generally requires two of these three findings:

1. Infrequent or absent ovulation, often reflected in irregular periods.
2. Signs of higher androgen activity, either visible symptoms or elevated levels on blood work.
3. Polycystic ovarian morphology on an ultrasound.

This is why PCOS can look different from one woman to another. Someone may have irregular cycles and elevated testosterone but a normal-looking ultrasound. Another person may have regular-appearing cycles yet clear androgen symptoms and polycystic ovarian morphology. Both may meet the criteria, depending on the full clinical picture.

The name can also be misleading. PCOS does not mean you have ovarian cysts in the way many people imagine. The ultrasound finding refers to a higher number of small developing follicles or increased ovarian volume. These follicles are not dangerous cysts, and an ultrasound is not required for every diagnosis.

The First Step: Your Health History and Cycle Pattern

A good appointment starts with questions, not a lab order. Your clinician may ask when your periods began, how often they come, whether your cycle pattern has changed, and whether you use hormonal birth control. They may also ask about pregnancy plans, medications, family history, weight changes, sleep, stress, and past blood sugar or cholesterol results.

For adults, cycles occurring fewer than about eight times a year, cycles longer than 35 days, or long stretches without a period can point to irregular ovulation. But cycle length alone does not tell the whole story. Some people bleed monthly without ovulating consistently, while others have irregular periods for reasons unrelated to PCOS.

Your provider will also ask about symptoms linked with androgens, the hormones often called “male hormones” even though everyone produces them. These can include persistent acne, increased coarse hair growth on the face, chest, or abdomen, and scalp hair thinning. Symptoms deserve to be taken seriously, but they do not automatically confirm PCOS. Genetics, ethnicity, skin conditions, medication effects, and other endocrine issues can all influence how androgen activity shows up.

If you are preparing for an appointment, a simple record can be useful: note the first day of each period, skipped cycles, changes in hair or skin, medications, and any relevant family history. This is not self-diagnosis. It is practical context that can help your clinician see a pattern more clearly.

Blood Tests Help Confirm the Pattern and Exclude Other Causes

There is no single “PCOS test.” Blood work is usually used to assess androgen levels and rule out conditions that can cause similar symptoms.

Depending on your history, a clinician may check total and free testosterone, DHEAS, thyroid-stimulating hormone, prolactin, and 17-hydroxyprogesterone. A pregnancy test may also be appropriate when periods are late or absent. These tests can help distinguish PCOS from thyroid disorders, elevated prolactin, nonclassic congenital adrenal hyperplasia, pregnancy, or less common androgen-related conditions.

The details matter. Testosterone tests can vary in accuracy between labs, and hormone levels may shift with time, medications, and hormonal contraception. Birth control can change bleeding patterns, suppress ovulation, and affect androgen testing, so ask your clinician whether testing while on it will answer the question you are trying to answer. Never stop a prescribed medication on your own just to prepare for lab work.

Because PCOS is closely associated with insulin resistance for many, though not all, women, clinicians may also assess metabolic health. This could include blood pressure, cholesterol and triglycerides, blood glucose, A1C, or an oral glucose tolerance test. A normal A1C does not always rule out early glucose regulation concerns, so the right test depends on your personal risk factors and your clinician’s judgment.

Metabolic screening is not about blame or a number on the scale. PCOS can affect people across body sizes, and insulin resistance can occur in people who are thin, athletic, or outwardly healthy. The goal is to identify useful information early and choose support that fits your actual needs.

When an Ultrasound Is Useful - and When It Is Not

A pelvic ultrasound may be part of a PCOS evaluation, but it is not a required first step in every case. If an adult already has irregular ovulation and clinical or lab evidence of higher androgens, an ultrasound may not change the diagnosis.

When used, the scan looks at ovarian volume and the number of small follicles. The exact thresholds depend on the equipment used and current clinical guidance, which is one reason a scan result should always be interpreted by a qualified clinician in context. Seeing polycystic ovarian morphology alone does not mean you have PCOS. Some women without the syndrome have this ovarian pattern and have no hormone or ovulation-related concerns.

An ultrasound can also be useful when a clinician needs to investigate pelvic pain, unusual bleeding, an ovarian mass, or another possible gynecologic explanation for symptoms. It is a tool, not a verdict.

Diagnosis Is Different for Teens and During Major Life Stages

PCOS can be particularly difficult to diagnose in adolescence. Irregular periods, acne, and multifollicular ovaries can all be common in the first years after a first period, even without PCOS. For that reason, clinicians are generally more cautious with diagnosis in teens and often focus on persistent menstrual irregularity and evidence of androgen excess over time. Pelvic ultrasound is usually not relied on early after menarche because it can be misleading.

Life stage also changes the conversation. During the postpartum period, breastfeeding can affect cycles. Perimenopause can bring irregular bleeding and shifting hormones. Fertility treatment, hormonal contraception, and certain medications can alter the information available from symptoms and testing. If your body has changed, your evaluation should account for that context rather than forcing your experience into a one-size-fits-all checklist.

What a Careful PCOS Evaluation Should Not Do

A diagnosis should not be based on an ultrasound alone, a social media symptom list, or the assumption that every irregular period is PCOS. It also should not be dismissed simply because your body size does not match a stereotype or because you are not trying to get pregnant.

You deserve room to ask direct questions. Which two criteria do I meet? What other conditions were ruled out? Are my cycles likely ovulatory? Which metabolic markers should we monitor, and how often? Clear answers can turn a vague label into useful information.

If your periods have stopped for several months, your bleeding is unusually heavy, you have rapid new facial hair growth or deepening of the voice, or you develop severe pelvic pain, seek medical care promptly. Those symptoms deserve timely assessment and should not be assumed to be PCOS.

A PCOS diagnosis is not a judgment and it is not a prediction of your future. It is a clinical framework that can help you and your care team make sense of what is happening now. Start with a qualified evaluation, bring your questions, and let the next steps be guided by your symptoms, goals, and health history - not by fear or guesswork.
